If you do not see your insurance listed above, please call our office to confirm that we are in-network.  If your insurance company (HMO) requires you to have a PCP (primary care physician) Dr. J. Clark Hill or Hill Medical Group must be listed on your card.  If this is not on your card, you will either need to reschedule your appointment until this is corrected, or you may pay for the office visit in full before being seen.

  • Beginning January 1, 2013, all copays and self pay office visit charges are due up front prior to services being rendered.  If you do not have your payment with you at the time of service, your appointment will be rescheduled.
  • Beginning January 1, 2013,  there is a $25.00 charge for missed appointments.  We make every effort to contact you with a reminder call before the day of your appointment.  If you fail to make a scheduled appointment, the missed appointment fee must be paid prior to your next appointment.
